Efficacité et tolérance du paclitaxel hebdomadaire dans le cancer du sein avec envahissement médullaire symptomatique

Abstract : Background: Bone marrow infiltration (BMI) is an uncommon event during breast cancer evolution. Few data are published and there is no recommendation on systemic treatment of this condition. In our center, breasts cancers with BMI are usually treated with weekly paclitaxel. The aim of our study was to evaluate the efficacy and safety of paclitaxel in this population. Methods: This retrospective singlecenter study included all breast cancer patients with BMI receiving paclitaxel between January 2014 and May 2018. The primary endpoint was overall survival (OS), secondary endpoints were progression-free survival (PFS) and safety. Results: BMI diagnosis was made in 26 patients and proven in 73% of cases with a blood smear test. Among these patients, 96% had bone metastases. At BMI diagnosis, 100% of patients had anemia and 77% had thrombocytopenia. OS and PFS were 7,2 months (95% confidence interval (CI) 2,6 – 20,7 months) and 3,3 months (95% CI 1,6 – 7,2 months), respectively. Good performance status (WHO 0-1), absence of thrombocytopenia and the presence of less than 5% of erythroblasts in peripheral blood at the initial diagnosis of BMI, were associated with better survival. Only one patient had a grade 5 febrile neutropenia, and no patient had any bleeding. Conclusions: Weekly paclitaxel is an effective therapeutic option with an acceptable toxicity for the treatment of breast cancer with BMI.
