Hémopathies malignes et exposition résidentielle environnementale aux pesticides agricoles en population générale : revue systématique de la littérature

Abstract : Over the last 50 years, incidence of Haematological Malignancies (HM) has been constantly increasing, concurrently with the expanding use of agricultural pesticides. The WHO classified some pesticides as carcinogenic or probably carcinogenic to Human, of which 6 are specifically associated with HM, mostly based on studies conducted among workers. Public debate on health impact of agricultural pesticides among neighbouring residents has been recently reopened, especially in France, which is the second consumer of pesticides in Europe. The objective of this thesis was to carry out a systematic review, documenting the link between environmental residential exposure to agricultural pesticides and HM risk in the general population. The originality of this work was to build a score appraising methodological quality of each included study, based on geographical subdivision, exposure assessment, outcome, exposure period and statistical procedure. Twenty epidemiological studies were included, of which 11 were ecological, 8 case-control and 1 a cohort, and of which 12 were conducted in children. These studies reported statistically significant positive associations between HM risk and some individual pesticides, pesticide classes or crop types. These associations were in line with those reported in the literature among workers or children of exposed parents. Nonetheless, results of the quality score were highly various, what reflected an important methodological heterogeneity across studies. Such studies should be proceeded with a standardization of the methods, especially concerning exposure assessment, in order to support Public Health programs and risk reduction measures.
