Le delirium en réanimation pédiatrique : traduction et mise en place d’une échelle diagnostique, étude de prévalence, usage et limites

Abstract : Objectives: in PICU, delirium has a prevalence of 10 to 44%. The Cornell Assessment of Pediatric Delirium (CAPD) is a validated scale for the detection of delirium in newborns and children up to 21 years old. The CAPD scale is not currently translated in French. The objective of this pilot study is to assess the use and relevance of a French version of CAPD. The prevalence of delirium and the factors associated with its occurrence are also studied. Material and methods: CAPD has been translated in French by reverse translation. For two months, all patients of the PICU were assessed by this scale every twelve hours. Caregivers were asked about the use of the scale and its relevance before and after the study. Results: 36% of the scales were completed. Over 80 patients, 38 had a CAPD score ≥ 9, resulting in a prevalence of delirium of 47.5%. 78% of caregivers thought that CAPD was useful in the service, 30% had trouble evaluating the patients. Children with cognitive impairment and infants were the most difficult to assess. Conclusion: this is the first evaluation of a French version of a diagnostic scale of delirium: the CAPD. The majority of caregivers had no difficulty using it and think that it is a relevant tool. The high prevalence shows that delirium is poorly known and underdiagnosed, requiring further studies in different centers.
