État des lieux de la fin de vie à domicile sur l’île de Mayotte, et attentes des professionnels de santé libéraux concernant l’EMSP

Abstract : Introduction: Mayotte, a French overseas département since 2011, is a developing département where living conditions have remained precarious. It is still a medical desert with home care well under national level. And yet it ranks as the French département where most people choose to die at home (56% of the deceased). Methods: with the first mobile palliative care unit (MPCU) being set up on the island in 2019, we decided to carry out an observational and descriptive study among private-practice nurses and general practitioners (GPs) , to identify and inventory their difficulties in providing health care to patients at end of their life at home, and to collect their expectations about this unit. Results: this study was carried out from March to June 2019; 73% of the nurses and 23% of the GPs of the island answered the survey questionnaire. Nurses report they feel alone and powerless (by lack of coordination with doctors or anticipation of the becoming of the patients’ health). They also report a shortage of other professionals (psychologists and social workers), but also technical difficulties (treatment arrangement). GPs report difficulties due to their workload (hence a lack of time to do home visits) but also problems in care coordination between hospital and home (inability to join hospital services and get hospitalization reports). Discussion: private-practice health professionals expect the MPCU to organize lectures (to help them improve their practice), to enhance communication with the hospital units, to improve social and psychological care and better communication with the patient and his family.
