Utilisation de l'immunothérapie par anti-PD-1 chez les sujets très âgés atteints de mélanome

Abstract : Introduction: we have previously studied safety of anti-PD1 treatment in elderly (over 80 years old) patients and reported a bicentric retrospective cohort showing similar safety profile in elderly and in younger patients. Quality of life evaluation data is still lacking in this specific population. Material and methods: prospective, single-center study including patients over 75 years of age with metastatic melanoma treated with anti-PD-1. Endpoint was to monitor quality of life (specific survey) and oncogeriatric assessment at the beginning of therapy, then 3 and 6 months after (nutritional status, comorbidities, autonomy, thymic, cognitive disorders). Results: fourteen patients were included from March to September 2018, median age 86.5 years [78-94]. General status was good, with a median Charlson score at 0[0-4]. Nine patients were evaluated at 3 months and six patients at 6 months. There was no significant difference in quality of life scores obtained at baseline and at 3 months and 6 months. QLQ-C30 score analysis showed a non-significant improvement in emotional and cognitive functioning. Discussion: this study showed that quality of life and autonomy does not seem to be affected by anti-PD-1 treatment in patients aged over 75 years. These results should be taken with caution, due to the low number of patients included, short follow up and monocentric data. Anyway, the prospective analysis and the complete oncogeriatric evaluation and monitoring represent unique and original data.
