Leviers et obstacles à la coordination du parcours de santé addictologique par les médecins généralistes du Marmandais : recherche qualitative par focus group et entretiens individuels semi-dirigés

Abstract : Introduction: pathological addiction is a brain dysfunction caused by a chronic consumption of psy-choactive substance. Addictions are major public health issues, with sanitary, social and medical con-sequences. The main objective of this work is to explore, through focus groups and semi-structured interviews, the representations, the reported practices of general practitioners in the Marmande’s health area re-garding the resources and means of addiction available in their sector. The secondary objective is to identify obstacles and levers affecting the use of these resources. Method: Qualitative study by focus groups and semi-structured interviews, carried out with 20 gen-eral practitioners in the Marmande health area, in Lot-et-Garonne (47). Results: general practitioners are at the centre of patient addiction management, particularly at the screening level, but there is a gap between practice and theory. The lack of time and training stand in the way of efficient care. Doctors’ representations about addic-tion will also slow down patient’s progress, potentially altering the necessary patient-carer relation-ship. The proposed levers are only starting to be implemented, but show promises. Conclusion: faced with the obstacles mentioned for the care of patients, the general practitioners were able to propose new potential approaches: increased training, development of mobile team to reduce isolation, strengthening existing care networks.
