Étude d’une prophylaxie quotidienne orale par vitamine C sur la récidive d’infections urinaires basses chez les femmes âgées résidant en EHPAD

Abstract : Urinary tract infections are very common in elderly population, especially women living in nursing homes. They have a direct impact on morbidity and mortality of this population with a significant socio-economic cost. Vitamin C has pro and antioxidant properties improving the fight against infection. The aim of our study is to evaluate the impact of empirical vitamin C prophylaxis (500 mg daily) treatment during 6 months on the occurrence of urinary tract infections in elderly women suffering from recurrent lower urinary tract infections living in nursing homes. The primary endpoint was the occurrence of urine culture-confirmed lower urinary tract infections during the study period. A prospective multicenter observational study was conducted from February 2017 to August 2017. Twelve patients were enrolled in two different nursing homes. One group already has vitamin C urinary tract prophylaxis in place and the other did not. After 6 months, the number of urinary infections was raised. At the end of the study, there was no statistically significant difference between the treated and control groups (RR = 1.89, 95% confidence interval [0.12-37], p value = 0.9). The lack of power of our study is its main limitation, future work on intention to treat with greater recruitment is needed to rule on the impact of vitamin C in the prophylaxis of urinary tract infections in elderly women living in nursing homes.
