Résultats à court et long terme et qualité de vie après duodénopancréatectomie totale : une expérience bordelaise à propos de 76 cas

Abstract : Objective: TP is an intervention known to have high post-operative morbidity and a significant impact on quality of life. The main objective of this study was to describe the short and long term morbidity and mortality of patients operated at the Bordeaux University Hospital of a TP, and to study their quality of life. Material and methods: this is a retrospective study of TPs carried out at the Bordeaux University Hospital between 2006 and 2019. Indication, co-morbidities and perioperative data were collected. An analysis of quality of life was done using the ADDQol, EORTC-QLQ-PAN26 and SF 36 questionnaires. PMSI data in France between 2010 and 2018 were analysed. Results: seventy-six patients were operated on by a TP at Bordeaux University Hospital during this period. The main indications were: pancreatic adenocarcinoma, renal cancer metastases, IPMN. Post-operative morbidity was composed of 92% of minor complications. The mean total length of stay was 17 days. There was only one death at the time of hospitalization. The majority of patients were satisfied with their medical management, and ADDQol had a mean weighted score of 0.15. Conclusion: the TP results were good in terms of morbidity and mortality of 60% and 1.3% respectively. From a distance, our results on the quality of life and the impact of diabetes are satisfactory. Today, TP is a safer intervention provided that the indications and patients are well selected and that preparation and very regular follow-up by a diabetologist is ensured. In view of our results, it seems quite reasonable to totalize partial pancreatectomy when the remaining pancreas is of "poor quality", or when the patient is already diabetic, in order to avoid serious complications of pancreatic fistula.
