Prévalence et facteurs associés aux infections à Treponema pallidum chez les patients consultant en centre de dépistage à La Réunion sur la période janvier 2017 à mars 2020

Abstract : Background : Syphilis is a sexually transmitted infection with an estimated prevalence of 0.5% worldwide in 2012. While syphilis in national and international literature is increasing in developed countries among men who have sex with men (MSM); the situation in Réunion differs. Objectives : To determine the prevalence of syphilis, its clinical characteristics and risk factors in a sexually transmitted infections testing centre in Réunion Island. Method : Cross-sectional, monocentric study based on clinical and biological data collected from all patients consulting between 2017 and 2020, through a standardized self-questionnaire and by performing syphilis serology. Results : Over the 3 years, 2593 patients were included. The prevalence of syphilis was 7.52% (n = 195; 95% CI 6.50-8.65) in the study population, 11.76% (n = 18; 95% CI 6.97-18.59) in young people and 36.36% (n = 16, 95% CI 21-59) in pregnant women. The risk factors identified in multivariate analysis were female sex (Odd Ratio (OR) = 1.85, 95% CI 1.10-3.11), age under 18 years (OR = 3.54, 95% CI 1.90 6.57), precariousness (OR = 3.12, 95% CI 2.11 4.62) and Reunionese origin (OR = 2.43 95% CI 1.42 4.13). The clinical description was varied (atypical ulcerations, multiple ulcerations, condyloma lata, syphilides,..). Conclusions : There is a high prevalence of syphilis in Réunion. Unlike in other developed countries, the main populations concerned are minors and women, raising concerns about a resurgence of congenital syphilis.
