Facteurs d'échec après reconstruction du LCA : comparaison entre échecs traumatiques et non-traumatiques

Abstract : Purpose: ACL reconstructions represent 49,200 acts in France in 2018. However, the reconstructions technical by intra-articular ligamentoplasty is not foolproof and there are, depending on the series, significant failure rates (1.8 to 17.7%). The objective of this study was to identify failures, analyze the causes and compare factors of failure. Materials and methods: 42 ACL revision, over a 5-year period at the Caen University Hospital, enabled a retrospective, comparative, monocentric and multi-operator study to be carried out. The patients' primary reconstruction surgery was then analyzed retrospectively. The patients were divided into two groups (traumatic and non-traumatic). Results: failures in 59.5% of cases were traumatic and 40.5% were of non-traumatic origin. The mean time before new ACL reconstruction was 4.3 years and it was significatively shorter in the non-traumatic group (p = 0.008) with 1.42 years vs 5.75 years. Patients with nontraumatic failure had a return to sport rate of 17.6% after reconstruction vs 91.7% for the traumatic group (p <0.0001). They also had a higher rate of the femoral tunnels malposition with 72% vs 29.4% (p = 0.005). The technical error rate was higher in the non-traumatic group (p = 0.5). Conclusion: technical causes, particularly femoral tunnel malpositioning, were significative correlated with increased incidence of non-traumatic ACL reconstruction failure, as well as a shorter average time before revision.
