La vaccination antigrippale modifie-t-elle le pronostic des patients âgés hospitalisés avec grippe ?

Abstract : Background: Few studies are conducted in older adults in order to determine if being vaccinated against influenza prevents from complications, despite the occurrence of influenza in a vaccinated patient. Our objective was therefore to determine if vaccination against influenza in the elderly reduces the risk of developing complications. Methods: We performed a comparative analysis of influenza cases in vaccinated and unvaccinated patients in an elderly population (> 75 years). Hospitalized patients from a national retrospective observational cohort were included during the 2016-2017 epidemic in France. Data were declarative. Immediate and 3-months follow-up outcomes were analyzed. Results: Among 515 patients, the vaccination status was known for 243 patients. 159 patients were vaccinated. There was no significant difference between the 2 groups for the baseline status. Only the number of comorbidities was significantly higher in the vaccinated group (Charlson index=2.2 in the vaccinated group versus 1.7 in the unvaccinated group (p = 0.02)). More than 50% of patients in each group presented one or more organ decompensations without any difference between the 2 groups. In-hospital outcomes were not different. At 3 months, the rates of mortality, rehospitalization, and admission to EHPAD were similar but the vaccinated group had a significantly higher level of functional recovery (ADL, 4.1 versus 2.3). Conclusion: Patients vaccinated against influenza and hospitalized had more co-morbidities than unvaccinated patients who were also infected. However functional recovery was better in these vaccinated patients.
