Abstract : The organization of unscheduled care in general medicine is a complex subject, carried out through regulation, sectorization and medical treatment by liberal general practitioners. The demographic and societal evolution of patients and practitioners leads to an increase in demand for care, a decrease in the number of practicing doctors, as well as more great difficulties in organizing a permanent ambulatory care, which leads to a transfer of patients to increasingly overcrowded Emergency departments. This situation invites us to think about new modes of exercise. The purpose of this study is to describe the unscheduled care supported by the offices of Amou and Pomarez. During the first half of 2016, the 8 doctors in this sector decided to stop the permanent care in effection from 8pm in the evening on weekdays and on weekends since Saturday noon. In return, a specialized doctor for intercurrent pathologies (MAPI) was available to take care of unscheduled care, without appointment, associated with a specific medical regulation telephone line. Analysis of the 3958 consultations carried out through information sheets pre-established during this period makes it possible to observe the characteristics of the pathologies observed and their care, in patients consulting particularly in the morning, of a comparable age with the local population. 73% of consultations were justified, and approximately 20% of patients would be allied to the emergency room in the absence of this "MAPI". This system allowed to avoid 585 inappropriate emergency departements admissions. Concerning the repercussions on emergencies of the end of the effection in the evening, and on weekends, the study model as well as the numerous biases inherent in this type of analysis do not allow us to conclude.
