Évolution de la fonction ventriculaire gauche après réentraînement intermittent en post-infarctus du myocarde : une étude en speckle tracking echocardiography

Abstract : Background: the improvement in maximal oxygen consumption (V̇O2peak) after cardiac rehab (CR) in myocardial infarction (MI) is usually explained by peripheral adaptations. However, the effect of CR on left ventricular (LV) function assessed by standard echocardiography remains unclear. The speckle tracking echocardiography (STE) detects precocious LV dysfunctions. The aim of this study was to analyse the effects of post-MICR (TG for trained group) after a 4-week High Intensity Interval Training (HIIT) on LV function assessed by STE compared to a control group (CG). Methods: measurement of the STE, standard echocardiography parameters and V̇O2peak was performed before and after CR in TG (n=20) and CG (n=18). Results: systolic global longitudinal strain (GLS) (-1.2%, p=0.035), early diastolic global longitudinal strain rate (SR-E) (+0.17s-1, p<0.001) and V̇O2peak (+3.48mL/min/kg, p<0.001) were improved after CR inside the TG and in comparison with GC (p<0.001, p<0.001, p=0.0012). Standard echocardiographic parameters remained. The improvement of V̇O2peak was correlated with improvement of GLS (r=-0.39, p=0.017) and SR-E (r=0.52, p=0.003). Conclusion: functional improvement after CR in post-MI is not only related to peripheral factors, but cardiac effects also play an important role.
