État des lieux de la prise en charge des grossesses prolongées au Centre Hospitalier Universitaire Grenoble Alpes : évaluation des pratiques professionnelles

Abstract : Purpose: the main objective was to perform a survey of the management of prolonged pregnancies at the Grenoble Alpes University Hospital. Methods: This is a monocentric retrospective observational study carried out at the maternity of the Grenoble Alpes University Hospital. Overall, 325 patients were included. Results: more than half of the patients with prolonged pregnancy (51.1 %) had induction of labor. 47.1 % of patients went into spontaneous labor and 1.8 % delivered by scheduled cesarean section before labor. Among the 325 patients included, 98.2 % were intending to treat “vaginal delivery”. 24.6 % of the patients delivered by cesarean section, 57.9 % delivered by spontaneaous vaginal delivery, and 17.5 % had instrumental delivery. Among the patients who went into spontaneous labor: 70.6 % delivered by spontaneous vaginal delivery, 17.6 % by instrumental delivery and 11.8 % by cesarean section. On the contrary, among patients with artificial induction of labor, less than half delivered by spontaneous vaginal delivery (48.2 %), 18.1 % by instrumental delivery, and 33.7 % by cesarean section. Conclusion: allowing monitoring of prolonged pregnancies up to 42 weeks of amenorrhea would reduce the number of patients with induction of labor, and thus the risks, complications and costs associated with this practice. In addition, the organization of consultation periods dedicated to the follow-up of prolonged pregnancies would help to relieve the pressure on the obstetrical emergencies while ensuring continuity of care adapted and respectful of the recommendations issued by the French National College of Gynecologists and Obstetricians in 2011.
