La télésurveillance clinique dans l’insuffisance cardiaque : intérêt d’un système de télésurveillance mis en place au centre hospitalo-universitaire de Bordeaux

Abstract : The aim of this thesis work is first to review the literature on telemonitoring in heart failure. Indeed, it has not yet proved its worth in terms of reducing hospitalizations and mortality, however, it has become increasingly important in our practices since the advent of the ETAPES project and since the health crisis caused by COVID 19. We therefore wished, through this work, to analyze the various studies on the subject, in an attempt to understand how telemonitoring can be used effectively, and in which patients. The second part is an observational study conducted at the Bordeaux University Hospital studying the relevance of a remote monitoring system used in the heart failure treatment unit. We analyzed the alerts generated and their relevance. This study shows that the Satelia system, which is deliberately designed to be intermittent and flexible, allows satisfactory compliance. The alerts are relevant in two-thirds of cases. Patients who do not generate alerts have few cardiovascular events, which tends to prove that the algorithm used and the frequency of queries (twice a week), do not miss important information and shows the relevance of the system.
