, Résumé Contexte: L'angor spastique est la traduction clinique d'un spasme d'une ou de plusieurs artères coronaires épicardiques. L'implantation de stent coronaire peut être une solution thérapeutique en cas de spasme coronaire focal réfractaire à un traitement médical optimal

, Objectif : L'objectif principal de cette étude est de définir le profil des patients ayant un spasme coronaire réfractaire et qui ont bénéficié d'une angioplastie par stent coronaire à l'Hôpital Cochin au cours des dernières années

. Méthodes, tous les patients souffrant d'un spasme focal réfractaire qui ont été stentés à l'hôpital Cochin ont été inclus dans cette étude rétrospective. Résultats: Au final 46 patients ont été inclus. Le sex ratio est de 1 avec un âge moyen de, vol.56, p.6, 2002.

, Les patients ont été admis pour un syndrome coronarien aigu (60,9%), dont 23,9% d'arrêts cardiaques ou une douleur thoracique atypique (39,1%). Les procédures d'angioplastie se sont déroulées par voie radiale (78,3 %), sur des lésions d'infiltration athéromateuse dans 84, Les facteurs de risque les plus fréquents sont le tabagisme (50%), l'hypercholestérolémie (39,1%), l'hypertension, vol.26, pp.12-44

, Il y a eu 27,3% d'événements ischémiques majeurs: huit revascularisations, un infarctus, trois accidents vasculaires cérébraux et un décès. Trois resténoses intra-stents ont été observées. Conclusion: L'angioplastie apparaît comme une bonne alternative thérapeutique dans les angors spastiques réfractaires lorsque le segment coronaire en cause est clairement identifié, avec une bonne efficacité sur les symptômes, été implantés avec succès
