Allergie à la viande de mammifères et morsure de tique : une nouvelle entité en Guyane Française ?

Abstract : BACKGROUND: The last decade has seen the emergence of novel form of delayed allergy after the ingestion of mammalian meat. This is mediated by an oligosaccharide ; alpha 1-3 galactosidase (alpha-gal). The objective of this study was to show the existence of this pathology in French Guiana and to describe the anamnestic, clinical and biological characteristics of these patients. METHODS: Patients describing an allergy to mammalian meat were included between September 2017 and August 2019. The variables examined included self-reported history of allergic reactions following ingestion of mammalian meat, mean time between ingestion and allergic reaction, tick bite exposure, history of atopy, levels of IgE antibodies to Alpha gal, and mammal meats. Results: 9/11 had digestive symptoms 4/11 had anaphylactic skin lesions, 3/11 had respiratory reactions, and angioedema. The delay between ingestion and reaction ranged from 1h30 to 6h. The offending meats were most often beef and pork, the eviction of mammalian meat stopped the symptoms. All patients were regularly exposed to tick bites. Of the 11 patients included, 7 were able to have a serum IgE antibodies, all serum analyzed (7/7) were positive for alpha gal. Conclusion: This study confirms the presence of this new entity in French Guiana and is the largest case-series in Latin America. It does not formally allow us to state that tick bites are the cause of this allergy, but all patients were regularly exposed to it.
