Étude du profil de sensibilisation à Aspergillus fumigatus d'une cohorte de patients adultes : apport dans le diagnostic et la prise en charge de l'aspergillose bronchopulmonaire allergique

Abstract : Allergic bronchopulmonary aspergillosis (ABPA) is an immuno-allergic disease primarily affecting patients with asthma or cystic fibrosis. Aspergillus fumigatus is the fungal species most frequently responsible of this disease. ABPA is a complex pathology that must be clearly differentiated from asthma with aspergillus sensitization because the complications and the prognosis are different.The prevalence of ABPA is 1 to 2% in asthmatics and 8 to 10% in cystic fibrosis patients. ABPA patients are known to be sensitized to the m3 source extract of Aspergillus fumigatus. Molecular allergology has made it possible to genetically engineer recombinant allergens, five of which are currently marketed: Asp f1, Asp f2, Asp f3, Asp f4 et Asp f6. The objective of our study was to find out whether these allergens could help in the diagnosis of ABPA and therefore in the management of these patients. We studied a cohort of 125 patients including 18 ABPA, 21 ABPA in remission and 86 non ABPA. We observed that ABPA patients produce IgE against several recombinants or even against all recombinants. The best performing tests require the use of all of these molecular allergens. In conclusion, our results suggest using all five recombinants for the diagnosis and monitoring of ABPA.
