Grossesse et transplantation rénale : étude de l’état psychologique de la femme enceinte porteuse d’un greffon rénal fonctionnel

Abstract : Objectives: Nowadays, kidney transplant is the best treatment for chronic kidney disease reaching its final stage and involves woman of childbearing age - between 20 and 44 years old. The main objective is to assess the psychological state of a pregnant women bearing a functional kidney graft. Secondary objectives are on the one hand to assess the quality of the support felt during the pregnancy and on the other hand to assess the impact (effects) of the kidney graft on the baby conception and progress with the pregnancy. Material and Method: It’s a quantitative descriptive prospective historical cohort study, carried out at the Grenoble Hospital by the mean of a form filled by women who had been pregnant during January 2005 and July 2017. The Zigmond and Snaith Hospital Anxiety and Depression Scale Psychometric scale, aiming at identifying an anxio-depressive symptomatology, has been used to analyse the women psychological state. Findings: Results showed that among the 34 pregnant women studied throughout the pregnancy, 65% registered anxiety, 18% registered depression against 50% of anxiety and 33% of depression in Bydlowski study in which the population was healthy. Specifically, 73% of anxiety secondary to the risk of renal dysfunction. The support felt is good with 95% claiming they have always been reassured by medical staff’s speech. Conclusion: Results corroborate that kidney transplantation act on the psychological state of a woman throughout her pregnancy and that the patient care should be specific. Future prospects are to extrapolate those results to other chronic diseases for pregnant women.
