Épidémiologie descriptive de la gale humaine au sein de la Province des Iles Loyauté de Nouvelle-Calédonie

Abstract : Background : Scabies is a public health problem in Oceania but no data exists in New Caledonia. This was a first study in the Province of the Loyalty Islands (PIL) of 20000 inhabitants. Method : We conducted a descriptive and exhaustive study with data from 1999 to 2018 using an algorithm to search for scabies cases in the electronic medical records (EMR) of primary health care center. Data validation, calculation of specific incidence by sex, age and census block, search for correlation with climatic and census data, and self-matching on < 5 years age class concerning skin infection before and one year after treatment have been performed. Results : 20988 scabies cases were detected by the algorithm with 100 % sensitivity and 94 % specificity. The average overall incidence rate was 6 % and 32 % in < 2 years age class. By the age of 14 years, more than 62 % of the children had experienced scabies. Women were more affected (SR=0.6). There is a strong seasonality (+30 % in winter), with an endemic trend. Geolocalization shows disparities in incidence but these disparities are not correlated with the average indicators of each geographical island concerning overcrowding, housing, or socio-economic level. After treatment, skin infections were 4 times less frequent in < 5 year olds. Conclusion : Scabies is highly endemic in PIL, as are neighbouring countries. It is responsible for many skin infections in children. Mass treatment could be considered. This study demonstrates the feasibility and power of the use of electronic medical records.
