Conséquences obstétricales et néonatales d’une prise de poids insuffisante pendant la grossesse chez la femme d’IMC pré-gestationnel < 18.5 kg/m² : étude de cohorte historique

Abstract : Objectives: This study aims to compare the maternal and neonatal outcomes of weight gain during pregnancy in underweight women (BMI < 18.5 kg/m²) according to the 2009 Institute Of Medicine recommendations. Methods: This is an observational, single-center, historical cohort study, conducted from January 1st, 2017 to May 31st, 2019 at the Couple Enfant Hospital of the Grenoble Alpes University Hospital. Two patient groups were formed. Inadequate group: underweight women with insufficient weight gain during pregnancy (< 12.5 kg). Appropriate group: underweight women with an appropriate weight gain during pregnancy (≥ 12.5 kg). The main outcome was birth weight for gestational age. Results: 135 women were included in the study including 59 in the inadequate group and 76 in the appropriate group. Birth weight for gestational age was significantly lower in the inadequate group compared to the appropriate group (30th percentile vs 49th percentile, p<0.0001) and 30.5% of the newborns had a birth weight for gestational age below the 10th percentile vs 10.5% respectively. Conversely, there was a higher rate of premature rupture of membranes in the inadequate group (39.3% vs 21.3%, p=0.02). Conclusion: Insufficient gestational weight gain in underweight women leads to a major risk of perinatal outcomes. Health professionals, in particular midwives and obstetricians-gynecologists, have an essential role in the medical care of underweight women.
