Prise en charge antalgique des patients traumatisés sur un domaine skiable : étude de la faisabilité d'un protocole de délégation de l'antalgie à des pisteurs secouristes pour les victimes d'un traumatisme sur un domaine skiable

Abstract : Introduction: Ski patrols are confronted daily with patients’ pain and often feel helpless. They currently do not have the possibility of offering analgesics to manage these pains. Doctors in Flaine have set up a protocol of analgesic delegation allowing first-aid trackers to offer paracetamol to trauma victims on the slopes. Materials and methods: Prospective comparative observational study on the winter 2018-2019. Main objective : Demonstrate the ability of trained first responders to apply a protocol for delegating analgesia. The respect of the protocol was evaluated by a group of experts thanks to the rereading of traceability sheets filled in by the ski patrols Secondary objectives : Evaluate pain reduction. Evaluate Patient and ski patrols satisfaction in their care. Results: 207 traceability sheets were reread by the expert group with a validation rate of 99%. There is a decrease of 1.33 in the pain scale between the beginning and the end of care. The overall satisfaction of patients and ski patrols collected through the questionnaires is good. Discussion: This study highlights few points in the protocol that need to be reworked. The quality of application by the first-aid ski patrols is good, demonstrating the possibilities of handling a simple analgesic after training. Some errors were made by the first-aid trackers but the risk of error seems rather low (< 5%). A retrospective study over several years could perhaps provide more details.
