Évaluation d’un protocole d’anesthésie sans opioïdes sur la stabilité hémodynamique préopératoire de chirurgie de résection pulmonaire par vidéo-thoracoscopie

Abstract : Background: opioid free anesthesia (OFA) is a developing procedure, but its impact on intraoperative hæmodynamic stability remains unknown. The main objective of this study is to compare the intraoperative hæmodynamic stability of patients between the OFA and the opioid anæsthesia (OA) protocols used in our department. Methods: a retrospective, open-label study was conducted in the Bordeaux University Hospital (France) between March 2019 and August 2019 on adult patients undergoing video-assisted thoracoscopic surgery (VATS) for lung resection. Subjects were divided in two groups: OA (sufentanil) and OFA (dexmedetomidine or clonidine). Intraoperative hæmodynamic data were extracted from the electronic health records used in the department. Results: out of the 99 patients involved, 58 were in the OFA group and 41 in the OA group. On average, the time spent with a heart rate between 45 and 90 bpm during surgery was of 94.9% (OFA group) and 88.9% (OA group), p = 0.10. The time spent with a mean arterial pressure between 60 and 109 mmHg during surgery time was 85.3% (OFA group) and 85.9% (OA group), p = 0.80. The vasopressor drugs, antihypertensive drugs and atropine requirements during surgery were comparable between the two groups. Conclusion: our study shows that, in patients undergoing VATS for lung resection, an OFA protocol results in similar hæmodynamic stability compared to an OA protocol, without increasing the risk of severe bradycardia nor the use of vasopressive or antihypertensive medication during the procedure.
