Adaptations pédagogiques et organisationnelles du stage ambulatoire de niveau 1 liées à la réforme du Diplôme d’Études Spécialisées de Médecine Générale : vécu et opinion des Maîtres de Stage des Universités en Gironde

Abstract : Context: since November 2017 with the reform of the 3rd cycle, general practice residents must realize their first ambulatory course during their 1st year of internship, knowing that it was inaccessible to the 1st semester previously. It was usually carried out from the 3rd semester. Objectives: the main objective was to collect the opinions and experiences of internship supervisors in Gironde on the new model of internships concerning the first ambulatory course. The secondary objective was to obtain their opinion on their 1st semester resident. Methods: this is a qualitative study carried out through semi-structured individual interviews. 15 internship supervisors supervising residents in first ambulatory course for over a year and for the first time 1st semester were interviewed from March to May 2018. They were stopped with the data saturation. A thematic analysis of the transcripts was carried out. Résults: this study highlighted the advantages and disadvantages of this new model from the point of view of the internship supervisors. Opinions are mixed: sometimes difficulties were highlighted. Some expressed no concern about receiving a 1st semester and were satisfied with the experience. They didn’t see any difference with the more advanced semester. Others said their mission as internship supervisor was more time consuming and laborious, with the 1st semester being judged to be less competent than previous residents. Empowerment was sometimes difficult if not impossible. Often, the impact was judged to depend on the resident and not on the semester Discussion: the internal validity of the study is ensured by a triangulation of the data with double coding The interviews were carried out early after the implementation of the the new model, which can explain some feelings. A consultation between the internship supervisors and the Département of General Medicine could be interesting in order to improve some feelings.
