Résumé

Introduction: numerous studies show that influenza vaccination coverage is insufficient in elderly subjects compared to national targets. Several solutions have been issued to reverse the trend. An argument is then of interest as a tool to help the doctor answer the patient's questions. Such a tool was proposed by the ARS. Objectives: The main objective of this thesis was to assess the impact of the arguments of the Hauts de France ARS in patients, over 75 years of age, refractory to vaccination. Method: qualitative study with thematic analysis resulting from semi-structured interviews in Gironde and Landes, from October 2019 to March 2020, in 14 patients over 75 years of age refusing to be vaccinated against influenza. Results / Discussion: the ARS argument alone was neither appropriate nor sufficient. The doctor emerged as the trusted person who could potentially convince most effectively with the help of this tool. The flu was underestimated with a feeling of immunity, not fragility. The vaccine, its composition, its adverse effects were feared by patients. The benefit appeared small. The main solutions put forward were the provision of appropriate information by a health professional to improve accessibility to vaccination. The COVID-induced situation had prompted only one in four patients re-interviewed to get the flu shot. Conclusion: this study has shown the limits of this tool which should be placed in the hands of a doctor and adapted to the patient. It also helped to better understand the reasons for refusing influenza vaccination. Other promotional tools or campaigns could then emerge as a result of this work.
Introduction : de nombreuses études montrent que la couverture vaccinale anti grippale est insuffisante chez les sujets âgés par rapport aux objectifs nationaux. Plusieurs solutions ont été émises pour inverser la tendance. Un argumentaire présente alors un intérêt en tant qu'outil pour aider le médecin à répondre aux questions du patient. Un tel outil a été proposé par l'ARS. Objectifs : l’objectif principal de cette thèse était évaluer l'impact de l'argumentaire de l’ARS des Hauts de France chez les patients, de plus de 75 ans, réfractaires à la vaccination. Méthode : étude qualitative avec analyse thématique résultant au d’entretiens semi dirigés en Gironde et dans les Landes, d’octobre 2019 à mars 2020, chez 14 patients de plus de 75 ans refusant de se faire vacciner contre la grippe. Résultats/Discussion : l’argumentaire de l’ARS seul n’était pas adapté, ni suffisant. Le médecin ressortait comme la personne de confiance pouvant potentiellement convaincre le plus efficacement avec l’aide de cet outil. La grippe était sous-estimée avec un sentiment d’immunité, de non fragilité. Le vaccin, sa composition, ses effets indésirables étaient redoutés des patients. Le bénéfice apparaissait faible. Les principales solutions émises étaient la délivrance d’information adaptée par un professionnel de santé, d’améliorer l’accessibilité à la vaccination. La situation induite par la COVID n’avait incité qu’une seule patiente sur quatre, réinterrogés, à se faire vacciner contre la grippe. Conclusion : cette étude a permis de montrer les limites de cet outil qui devra être placé entre les mains d’un médecin et adapté au patient. Elle a aussi permis de mieux comprendre les motifs de refus de la vaccination antigrippale. D’autres outils ou campagnes de promotion pourraient alors émerger suite à ce travail.
