Évaluation de l’apport de la télémédecine à la régulation du SAMU lors de l’expérimentation d’un projet de télémédecine pré-hospitalière sur le secteur de Nontron en Dordogne

Abstract : Introduction: the Nontron area, in the Dordogne, has become a medical desert. The Health Regional Agency, the Emergency Medical Help Service and the local Fire Department, put together a trial of telemedicine in a coach. The Nursing Emergency Vehicle (VISU) well stocked with medical and electronic devices (camera, electrocardiogram) allows informations transfer to the doctor in charge. Our study will try to see if informations collected through "telemedecine" tools are influential on the reasoning and final decision of the doctor. Methodology: this work is the first observational, descriptive and retrospective study on the subject. All the interventions of the VISU between of the first January to the thirty October are included. The primary endpoint was to determine if the teletransmeted observations were allowing a better and more human first intervention. Secondary endpoints are based on the descriptions of the various situations. Results: on the 137 VISU interventions, a change of decision in 83 cases (61,0 %) is noticed with a significant rise of the reply level. 8,0 % patients stayed at home and 10,0 % were directly sent to specific units to hospital. VISU went out frequently in the most populated areas, for heart and traumatological cases. Most of the patients have been sent to Périgueux hospital. Conclusion: the VISU coach allowed a noticeable improvement in the first pre-hospital assessment of the patients situations. Telemedecine seems to bring important information helping to improve the quality of the reply to many situations. These results should be confirmed in the future through other studies.
