Efficacité du système FreeStyle Libre® dans une population de 668 patients atteints de diabète de type 1 ou 2, traités par multi-injections ou pompe à insuline‎ : analyse d’une cohorte mono-centrique‎

Abstract : Introduction: The FreeStyle Libre® (FSL) system is reimbursed in France for patients with T1D or T2D treated by several insulin injections or insulin pump. Prospective studies mainly showed a reduction in hypoglycemia risk. However, the effect on HbA1c remains controversial. The purpose of our study was to assess the FSL effect on HbA1C in a population of T1D and T2D treated by multiple daily insulin injections (MDI) or continuous subcutaneous insulin infusion (CSII) in a real-world setting. Materials and methods: We conducted a retrospective, monocentric, cohort study, including T1D and T2D patients who received a specific FSL training between June 2017 and June 2018 in Caen University Hospital. The primary endpoint was variation of plasma HbA1c (∆HbA1c) from baseline (within 3 months before FSL introduction) to outcome (within one year after FSL introduction). Secondary endpoints were mainly CGM parameters and were analyzed 3 months after FSL introduction. Results: 445/223 DT1/DT2 were included: average age 45.8/65.0 years; women 49.1/39.0%; duration of diabetes 20.1/20.6 years; CSII treatment 68.8/56.0%; baseline HbA1c 7.9/8.2% (p = 0.017) and ∆HbA1c -0.4/-0.6% (p = 0.055), respectively. ∆HbA1c decrease was significantly more important for patients treated by MDI than those treated by CSII in T2D (-1.0% and -0.4%; p = 0.014). In T1D, the improvement in glucose parameters was correlated with the number of scans. In T2D, only the ∆HbA1c was correlated with the number of scans. Conclusion: This real-life cohort study highlights a positive effect of FSL on HbA1c in T1D and T2D patients whether treated by MDI or CSII.
