Évaluation clinique de l’efficacité des infiltrations échoguidées du mur méniscal

Abstract : Introduction: the meniscal syndrome is a very common cause of gonalgia. Ultrasound (US)-guided injections of the meniscal wall have shown good results, but they still remain poorly prescribed as symptomatic medical treatment. We conducted an open, retrospective and monocentric clinical study to evaluate the efficiency and safety of these injections. Methods: the population consisted in 35 consecutive patients who had a US-guided injection of the meniscal wall between the 01/01/2017 and 10/31/2018 at Sainte Marguerite Hospital (APHM). The primary outcome was the effectiveness of infiltration on pain by comparison of the verbal 11-point Box Scale (Verbal BS-11) before and 6 weeks after infiltration (collected by phone questionnaire). Secondary outcomes were physical activity recovery, improve of knee flexion, occurrence of complications and pain felt during the injection. Results: a statistically significant efficiency on pain was found, with an average of Verbal BS-11 of 6.9 (+/- 1.6) before, lowered to 2.2 (+/-2.5) after infiltration, p<0.0001. We also found a positive impact of the infiltration on the function (increase of the sport practice and the range of motion), without serious undesirable effect identified. Conclusion: this infiltration appears effective and interesting as first-line symptomatic medical treatment, for patients suffering from uncomplicated meniscal syndrome.
