Diagnostic précoce de dengue : étude de la performance du TROD SD Bioline Dengue Duo sur l’île de la Réunion

Abstract : Introduction: Dengue is a major public health issue with more than 55% of the global population exposed. In Reunion island an outbreak occurs since 2018. The health care facilities are facing the problem of managing a massive influx of patients and increased care burden. The aim of this study was to evaluate the performance of RDT SD Bioline dengue duo on the adults patients consulting at the emergency department during the 2019 epidemic. Methods: This retrospective cohort study included patients over 18 years, suspects of dengue, who consulted between the January first and June 30th 2019 in Saint-Pierre emergency department, and were tested with RT-PCR dengue and RDT. Results: 2021 patients were screened retrospectively during the study period. 321 patients matched inclusion criteria. The NS1 component had a sensitivity of 48% (IC95% 36-60) and a specificity of 58% (IC95%: 50-67). The IgM component was highly sensitive 86% (IC95% : 79-94), but not specific 6% (IC 95% : 3-9). There was no improvement of RDT performance to the early-stage (within the first 5 days) compared to the late-stage (beyond the 5th day). Conclusion: The SD BIOLINE Dengue Duo RDT showed low sensitivity and specificity. These data suggest it is not effective to rule in or rule out the dengue diagnosis in emergency department during an outbreak.
