Diabète de type 2 et pied diabétique en Nouvelle-Calédonie entre santé publique et culture : une approche de promotion de la santé pour la prévention des amputations des membres inférieurs chez les personnes diabétiques

Abstract : Introduction. – Although the Diabetes program of the New Caledonia’s Health and Social Agency has been deploying a podology system for diabetic person since 2008, the incidence of lower limb amputations has not decreased. The aim of this study was to understand the non-decrease in the number of amputations despite the implementation of this system. Thus, it was important to identify the determinants influencing the prevention of diabetes and its complications, but also to know the sensitivity of health professionals to the diabetic foot and its podological risk, as well as intervention’s modalities of the 3 actions of the system. Material and method. – Semi-directive interviews were conducted with 7 diabetic person (including one amputee) and 9 health professionals (3 podiatrists, pilot doctor of the Diabetes program and 5 medical health professionals). They were supplemented by a study of documents and direct observations of the 3 actions of the podology system. Results and discussion. – As in the literature, many social determinants of health were implicated in diabetes and its complications in New Caledonia. Concerning the diabetic foot, facilitating factors and obstacles were highlighted at the level of diabetic person, health professionals, the podology system and the New Caledonian environment. Conclusion. – Preventing lower limb amputations in diabetic people could not be limited to foot care alone. Thus, through the results of the study, the literature and knowledge of New Caledonian territory, a set of recommendations were proposed in order to improve diabetes prevention and its management according to a health promotion approach.
