Maladie thromboembolique veineuse chez les patients atteints de cancer au CHU de Caen : épidémiologie, prise en charge et complications

Abstract : Introduction: Venous thromboembolism (VTE) is a common complication in patients with cancer. These patients need specific care. Objectives: To describe the epidemiological characteristics and therapeutic management at Caen University Hospital for the patients with cancer associated thrombosis. To describe the main complications that occurred, the follow-up, and to analyze the 1-year survival. Material and methods: Patients with cancer associated thrombosis at Caen University Hospital from October 2018 to March 2019 were included retrospectively. Age, sex, history, cancer, thrombosis sites, therapeutic management, complications, follow-up and 1-year survival have been analyzed. Results: Among 112 patients with cancer associated thrombosis, 58 (52%) had pulmonary embolism (PE), 38 (34%) had deep vein thrombosis (DVT) without associated PE, 7 (6%) had isolated superficial vein thrombosis (SVT), and 9 (8%) had isolated venous thrombosis in unusual sites. Patients with pulmonary cancer had a significatively higher rate of PE (79%) than patients with non-pulmonary cancer (p=0,003). 95 patients (85%) were treated with anticoagulants at curative dose, 4 (4%) received a vena cava filter, 8 (7%) were treated with anticoagulants at preventive dose and 5 (4%) did not receive any specific treatment. 26 patients (23%) had a bleeding event, and 6 patients (5%) had VTE recurrence. The 1-year survival rate was 46%. There was no significative difference in survival rates between patients with PE (48%) and patients with another VTE events (44%). Conclusion: VTE is still a major cause of morbidity in patients with cancer. Complication rate associated with VTE is still significant in this population. Few patients are treated with direct oral anticoagulants. This matter should continue to be studied in the coming years.
