États mentaux à risque de transition psychotique : revue de la littérature en France et à l'étranger

Abstract : Since the 1990's, early intervention in psychosis, especially in first episode, has been widely developed in the world. The interventions are not as well codified regarding the at risk mental state for psychosis. Our work was a literature review, which aimed to describe the current state of affairs about early intervention in at risk mental state, at international level and in France. We showed that early intervention services for psychosis were now well established in the western world, particularly in Anglo-Saxon and Scandinavian countries, as well as in Western Europe and in some Asian areas. Several national or regional networks have been created; they coordinate the implementation of clinical services and edit guidelines. In France, the very first programs were born approximately twenty years ago, but they extended belatedly in the country. However, their number are now growing quickly since the last years, despite many obstacles at economical, organisational and scientific levels. Furthermore, the at risk mental state approach is evolving thanks to new diagnostic and therapeutic strategies, for example the establishment of youth mental health services (like the Maisons des Adolescents system) in several countries. At the end of this work, it seems manageable to implement widely early intervention programs for at risk mental state in France, in an effective and acceptable way.
