Évaluation de la survie des patients du Finistère, atteints d’un carcinome épidermoïde de l’œsophage et de la jonction oeso-gastrique, au stade métastatique, de 2009 à 2018. Étude ESPErE

Abstract : This study was conducted to evaluate de survival of patients with metastatic esophageal squamous cell carcinoma (ESCC), treated in the Finistere area. Materials and Methods : patients ≥18 years old, inhabitants of Finistere, with CEO or CEJ, diagnosed at the metastatic stage between 2009 and 2018 were included. Demographic and therapeutic data were retrospectively collected. A comparative analysis was performed between patients receiving systemic palliative treatment (TSP) and patients in best supportive care (BSC); among the TSP group, between patients who had received prior treatment for localized disease (pre-treated group) and those who metastasized from the outset (naïve group). The primary endpoint was overall survival (OS). Results : altogether, data from 341 patients were analyzed retrospectively. The median OS for the total population was 5.8 months [95% CI: 5.027-6.998]. The OS at 1 year and 5 years was 24.3% and 2.7%, respectively. OS in the BSC group was 1.7 months [95% CI 1.35-2.27] compared to 9.13 months in the TSP group [95% CI 7.98- 10.12]. In the TSP group, no significant difference in OS was demonstrated between the pretreated and the naive patients. Participation in a therapeutic trial was a predictive factor of survival (p = 0.015). Conclusion : ESCC remains a cancer with a very severe prognosis. Survival declines rapidly as the therapeutic sequences progress, whether or not the patients have received previous treatment. The impact of clinical trials, especially immunotherapy, from the 1st-line is currently being evaluated with encouraging preliminary results so as to change the current prognosis of this pathology.
