Rendement diagnostique et critères prédictifs des biopsies bronchiques écho-guidées par mini-sonde dans le cadre des lésions pulmonaires périphériques : étude mono centrique rétrospective au CHU de Caen

Abstract : Introduction: Radial echo-endoscopy is an endoscopic technique that allows to locate peripheral lung lesions in real time in order to take samples. The main objective of this study was to evaluate the diagnostic performance of the radial echo-endoscopy in our center and to determine predictive factors of diagnostic effectiveness. Methods: This is a monocentric retrospective study of patients with biopsies after mini-probe locating between April 2019 and July 2020 at the Centre Hospitalier Universitaire de Caen. Results: Over this period, 66 patients were sampled after being located by miniature probe. A diagnosis was possible in 56.1% of cases, of which 78.4% were pulmonary neoplasia. Analysis of PDL1 was possible in 73.9% of cases. Mutation testing was performed in 76.4% of adenocarcinoma cases. Factors influencing the diagnostic profitability were: the presence of a sign of the bronchus on the CT scan (p=0.005), the proximity of the lesion (at or before the third bronchial division) (p=0.014), a circumferential ultrasound image (p=0.022). Conclusion: Radial echo-endoscopy is a effective diagnostic examination for the characterization of peripheral lung lesions provided that the eligible lesions for the technique are properly characterized.
