Prise en charge par Colchicine d’enthésites résistantes aux traitements antalgiques : étude descriptive sur une cohorte de 31 patients

Abstract : Background: Colchicine is one of the first-line treatments used in the management of gout attacks. Our study focused on a population of patients referred for Rheumatology / Algology consultation for enthesitis resistant to conventional analgesic treatments. These patients were then effectively treated with Colchicine under the assumption of a gouty etiology to their pain. We then carried out a descriptive study of these patients. Method: Retrospective, descriptive, and single-center cohort study on 31 patients Results: The descriptive study on our 31 patients cohort showed an average age of 61,387 years old. The mean pain duration was 30 months. 10 of them were men and there were 21 women. Regarding their medical history, 22 patients used to suffer from high blood pressure, 17 patients used to suffer from overweight or obesity, 16 used to suffer from type 2 diabetes and 13 used to suffer from chronic kidney disease. 14 patients had a rheumatic disease history and 13 patients had a gout family history. The results of the blood test showed a mean uricemia of 417,81 μmol/L, the mean serum creatinine level was 85,69 μmol/L, the mean PCR level was 5,16 mg/L and the mean monocyte level was 659,03/mm³. The radiographic examinations found signs of enthesopathy for 26 patients. Conclusion: The characteristics described in the patients of our population led us to link this table of enthesitis to a gouty origin. We have therefore proposed to qualify this clinical picture by the term “Enthesitis with hyperuricemia”. The knowledge of these clinico-biological characteristics could help primary care practitioners in the management of this type of patients.
