Étude de l’exposition médicamenteuse aux cardiotropes et aux psychotropes des sujets âgés consultant aux urgences pour chute avec malaise en Rhône-Alpes

Abstract : Objective: Polypharmacy and the use of cardiovascular agents and psychotropic drugs are risk factors for falls in the elderly. The main objective of this study is to demonstrate an increased drug exposure to cardiovascular agents and psychotropic drugs in elderly subjects consulting for a fall with syncope in the emergency room. Methodology: Our multicenter and quantitative study included subjects aged 75 and over, admitted in the emergency department for falls, based on evaluation data of professional practices carried out in the Nord Alpine region by the French Network of North-Alps Emergency Departments (Réseau Nord Alpin des Urgences, RENAU). The patients included were divided into 4 groups: "syncope", "accidental falls", "repeated falls" and "other types of fall". From the emergency room admission prescriptions, we studied the consumption of psychotropic and cardiovascular agents in number and quality in the "syncope" group compared to other types of falls. We were interested in inappropriate drug prescriptions of cardiovascular agents and psychotropic drugs, defined according to the The French Health Authority (Haute autorité de santé or HAS). Results: We included 862 patients. In the “syncope” group, comprising 98 patients, the average number of cardiovascular agents was 2.28 (σ = 1.68) and was significantly higher (p <0.01). The average number of psychotropic drugs in the “syncope” group is 1.04 (σ = 1.23) with no significant difference with other types of falls (p = 0.77). No significant difference in inappropriate drug prescriptions of cardiovascular agent was found between the groups. Overexposure to inappropriate medication prescriptions of psychotropic drugs was found in the “repeated falls” group (p <0.01). Conclusion: This study shows overexposure to cardiovascular agent in elderly subjects admitted in the emergency department for falls with syncope. No increased exposure to psychotropic drugs was found in the "syncope" group compared to other types of falls.
