Phase d'atténuation de la pandémie à coronavirus SARS-CoV-2 et SOS Médecins Caen : étude observationnelle prospective de la gestion d'une crise sanitaire par une association de permanence et de continuité des soins ambulatoires

Abstract : Introduction: the SARS-CoV-2 pandemic caused an unprecedented health crisis for France. Structural and organizational changes in the ambulatory and hospital care systems were necessary to overcome with this. Methodology: this descriptive, prospective and monocentric study was conducted in the department of Calvados. It concerns all the acts carried out by SOS Médecins Caen from March 17, 2020, to May 11, 2020, dates of the general lockdown of the French population. Covid-19 related actions were assessed, including daily activity, peak activity and suspicious cases characterization. Results: 13 425 acts were recorded, including 2 045 acts related to Covid-19, their hospitalization rate was 3.6%. It appears that the peak daily activity was reached on March 28, 2020 with 153 procedures and that it anticipates hospitalizations peak. A consultation center dedicated to patients suspected of Covid-19 and the development of teleconsultation were quickly set up. The proportion of overall activity represented by home visits, maintained 24/7 as part of the continuity of care, was increased by 21.6% and 45% for procedures performed in EHPAD compared to the previous year. Furthermore, the proportion of activity linked to chest pain and anxiety disorders was significantly increased over the period and appear to be linked to lockdown. Conclusion: this study confirms the role and adaptive capacity of SOS Médecins Caen in the outpatient health monitoring system and the ongoing struggle in the face of a health crisis to preserve the mobilization of hospital facilities.
