Activité de dermatologie d'un interne de médecine générale en SASPAS

Abstract : Introduction: a third of the French population would have a skin problem. Dermatology is a common discipline in general practice, and is often source of difficulties and uncertainty. Due to a decreasing number of dermatologists in France, general practice is more and more likely to be confronted to skin issues. The main objective of this thesis is to know which are the most frequent and the most doubtful pathologies and which diagnosis means are at our disposal. Methods: we carried out a prospective, observational study during the last semester of a general medicine residency in the Alpes-Maritimes, with a collection of dermatology cases encountered during current practice. Results: cutaneous disorders were found in one third of the consultations. The most common pathologies were wounds, benign and malignant skin tumors, eczema and herpes. The skin problems causing the most uncertainty were mainly the pigmented lesions. Discussion: our study showed that dermatology was frequent in general practice. Skin tumors are a highly prevalent reason for consultation, either for diagnosis or follow up. They fit perfectly into the prevention role of the physician. A consultation dedicated to prevention could free up time for skin monitoring of the patients. Caring skin tumors could be improved by setting up inter-professional support networks. Tele-expertise tools are developing, as are FMC training courses for post-graduate learning. Conclusion: dermatology is frequent in everyday practice. To help difficulties, tools are being developed for medical decision support.
