Effets de la réduction des heures de garde sur la formation et la qualité de vie de l’interne d’anesthésie-réanimation : une revue systématique de la littérature

Abstract : Objective: to evaluate the impact of restricting duty hours on anaesthesia and intensive care residents’ quality of life, quality of education and quality of care. Method: an electronic article search was conducted in the medical literature in three databases: Medline, Embase and Cochrane. The inclusion criteria were a population of residents rotating in anaesthesia, resuscitation or emergency medicine with duties including nights. The studies had to evaluate the impact of restricting duty hours on the quality of life of the residents, and/or the residents’ education and/or the quality of care. The quality of the articles was evaluated using the Medical Education Research Study Quality Instrument score (MERSQI). A qualitative analysis was performed. Results: of the 1604 articles identified, the final number of articles included in the systematic review was sixteen. Eight studies in total evaluated the impact of restricting duty hours on education. Two studies find a better quality of education after the implementation of the restricted duty hours. Naps during teaching and the attendance at educational activities were significantly reduced with the restricted duty hours. All the study results showed an improvement in the residents’ quality of life after restricting duty hours.Conclusion: regarding their education, residents are generally satisfied with the implementation of restricted duty hours. The study shows an improved state of health among residents with less fatigue and burnout.
