L’engagement des internes dans le programme « MBSR » (pour mindfulness-based stress reduction) proposé au centre hospitalier universitaire de Bordeaux : une approche qualitative

Abstract : Tools to prevent troubles linked to psychosocial risks in healthcare residents seem to be lacking. However, interventions based on mindfulness meditation such as the « mindfulness-based stress reduction » (MBSR) program have proven their effectiveness among such troubles. Therefore, it seems appropriate wanting to deliver this type of program to residents, as it has been experienced and positively reported in medical literature in recent years. However, the residents’ commitment into such a program sometimes seems limited. A complementary approach of the evaluation about the program’s theoretical effectiveness seemed appropriated, among the specific context of the program, as offered by the University Hospital Center of Bordeaux, in order to explain the factors influencing this commitment. We led individuals semi-directive interviews with residents who participated, planned to participate or didn’t plan to participate in this program to investigate such dimensions of commitment. The thematic analysis of these interviews’ content revealed five major themes, considered as majorly influencing this commitment: the program environment, the perceived benefits of the program, the knowledge and experience the resident links to the program, other facilitators or brakes having internals origins and other facilitators or brakes having externals origins. We tried to make a model of the influence of these factors in the form of three “ideal types”: the refractory, the hesitant and the committed. All the identified factors interact with each other, and cannot be considered out of the context in which they exist. Hence, it seems necessary to consider at the same time, according to the means the team has, organizing this kind of intervention. Dedicated prior information campaigns, as well as an a posteriori evaluation of the steps in this work, seem to cover the major part of the reported elements by the residents surveyed.
