Polyarthrite rhumatoïde et facteur rhumatoïde : comparaison de méthodes de dosage immunologiques

Abstract : One of the main issues in the management of rheumatoid arthritis is to set up the diagnosis at an early stage of the disease. The ACR/EULAR 2010 criteria were developped for that purpose. The weight of autoantibodies and in particular rheumatoid factor is important in those critera. Despite the existence of international standard used to calibrate rheumatoid factor assays, the polyclonal immune response, the diversity of autoantibodies produced, and the intrinsic differences between assays can lead to a heterogenous reponse between assays in the same patient sample. Through a population for whom rheumatoid factor and anti-citrullinated protein antibodies assays were performed at Caen University Hospital between 2015 and 2017, we compared the detection capacities of different rheumatoid factor assays including an "in-house" ELISA and their diagnostic performances in rheumatoid arthritis. The results of our study revealed differences in agreement between the different tests, the best agreement being observed between the immunoassays. Variations in the diagnostic performances were also observed, the "in-house" ELISA being the one with the highest sensitivity (60%) but at the expense of lowest specificity (69%). Despite those differences, some assays showed relevant diagnostic performances, such as Bioflash. Additional studies would be of interest to confirm those observantions.
