Caractéristiques et facteurs de risque d’hospitalisation des patients se présentant aux urgences du CHU de Caen pour suspicion d’infection au SARS-COV-2 lors du 1er confinement : étude monocentrique rétrospective

Abstract : Introduction: The world has faced a novel infection disease outbreak, due to a novel coronavirus, the severe acute respiratory syndrome coronavirus (SARS-CoV2), with a massive impact on healthcare ressource. Most COVID-19 infected patients present with mild to moderate acute respiratory symptoms. About 20% will present with severe or critical manifestations of the disease. Anticipating the need for hospitalization and intensive care is crucial. Emergency departments are at the front line for triaging patients with respiratory symptoms and referring them to the appropriate level of care. Méthode: This retrospective, monocenter study describ patients with a suspicion of COVID-19 infection admitted to the ED of CHU-Caen between March 17 and April 12. Vital signs, clinical and biological parameters and radiology findings have been collected as well as RT-PCR COVID result and any of the followings: simple ED visit, hospitalization, hospitalizatio in ICU and deaths. Resultats: 433 patients have been suspected of COVID-19 infection, among them 56 were COVID+. 58,9% of infected patients have been hospitalize, 21,4% in ICU and 3,6% have died. Most of hospitalized patient were men (67,3%), with high blood pressure (45,4%). They had a signnificativelly high respiratory rate (20c/min), and auscultatory crackling (54,5%). About the blood sample, they had a significativelly higher CRP rate and D-dimeres rate. Chest X-Ray was significativelly normal among those who were released home. Conclusion: Our study find some clinical signs that already have been described in literature such as respiratory symptoms, digestive symptoms or fever. CRP , D-dimeres as well as chest X-ray appear to have an interest to evalute the severity of this infection.
