Étude d’impact de la formation SANTÉ SUD « Urgences en centre médical isolé à Madagascar »

Abstract : Introduction : Santé Sud supports the development of community medicine to promote access to quality primary care for rural populations in Madagascar. The main objective of our study was to evaluate the impact of the training "Emergencies in isolated medical center", delivered to Malagasy community doctors (MC) in 2018. Material and Method : This was a prospective, multicenter, quantitative and descriptive study, conducted in Madagascar in 2019, covering 245 emergencies. The characterization of emergencies was based on the vital distress presented or the trauma context. Results : Nearly one in five treatments is not considered satisfactory, primarily because of a lack of availability of medicines. The conditioning is performed satisfactorily in the majority of cases, reflecting the progress made by the MCs who received the training. Lack of equipment is still a limiting factor for good practice. Nearly a quarter of patients have difficulties in developing diagnostic hypotheses. Respiratory distress is mainly represented, in connection with the acute respiratory insufficiencies of the child. Conclusion : The difficulties of CM in the diagnostic decision constitute an obstacle to the introduction of targeted and effective therapy. A lack of training, the impossibility of using a specialized opinion, the lack of accessibility to imaging or biology exams can explain these difficulties. The training developed by Santé Sud has improved the quality of care delivered in emergency situations. Satisfaction with the quality of care is important for each stage of the care strategy. The technical gestures are carried out successfully in the majority of the cases. The impact of the training "Emergencies in isolated medical center in Madagascar" is retained as very positive.
