Prise en charge de l’état dépressif dans la maladie de Parkinson : mise au point : revue de la littérature

Abstract : Introduction: Parkinson's disease is an idiopathic neurodegenerative disease expressed by motor and non motor symptoms. Some of these symptoms can mask a concomitant depressive state. The management of the depressive episode in the parkinsonian patient remains insufficiently studied with contradictory data and based on limited levels of evidence. Objective: to propose a review of the literature on the management for depression in Parkinson's patients, detailing diagnostic and therapeutic tools. Method: literature review based on the PubMed database from 1993 to 2019 concerning: Parkinson's disease, depression, confounding symptoms, rating scales and treatments. Results: The review is based on 50 studies. MADRS and GDS-15 can be relevant diagnostic tools. SSRIs can be recommended as the first line of treatment. IRSNa can be the second line, but must be used more carefully. Tricyclic antidepressants are not favored on the front line because of their anticholinergic effects. In case of resistance to antidepressant treatments, recurrent depression or partial remission, potentiation of antidepressant treatment with lamotrigine may be considered. Conclusion: certain tools can facilitate the diagnosis of a depressive episode concomitant with Parkinson's disease. Certain treatments are relevant in the presence of these two pathologies. More studies are needed to support the diagnostic and therapeutic data.
