Utilisation de l'électrocardiogramme chez les patients dans une maison de santé pluriprofessionnelle en milieu isolé

Abstract : The electrocardiogram is a reference for medical practice, whose current use in private practice is arbitrary despite the aging of the population and the increasing prevalence of chronic pathologies. Objectives: to cary out an assessment the use of the ECG in patients from a health center in an isolated environment. To determine the improvements and highlight the contributions of ECG use. Method: single-center observational prospective study of 3 months and analysis of each item of the questionnaire filled in by the general practioners of the health center after following an ECG. Results: 89 ECGs were performed during the study period, with an average of 1 ECG/physician/week and an average consultation time of 26 minutes. 57.3% of the patients were older than 50 years and the proportion of men and women was similar. About half of the patients who had a previous ECG could not provide information about it. 43 patients had more than 2 cardiovascular risk factors. The majority of consultations were for acute cardiological symptoms (mean age, 60.7 years), followed by certificates of no contraindication to sport (CNCI) (mean age, 33 years). Only 9 ECGs were abnormal, of which: 2 patients were referred to the emergency, 7 to a cardiology consultation and went home. A total of 5 patients were referred to the emergency, 15 to cardiology and 69 patients were not addressed to specific care. Approximately 1/3 of patients had treatment initiated following the consultation. Discussion: the ECG performed in the health center mainly concerned acute cardiological symptoms and certificates; their performance allowed us to decide on an appropriate management and referral, limiting the use of the emergency room. It is necessary to insist on certain notions such as the integration of the ECG in the initial assessment and follow-up of certain pathologies and treatments, the benefit to have a reference ECG and to increase awareness of the patients of their own cardiovascular risk.
