Placenta accreta : éléments prénataux associés à des complications maternelles postnatales du traitement conservateur pour une mauvaise insertion placentaire

Abstract : Objective : To determine prenatal elements associated with postnatal maternal complications of conservative treatment in cases of abnomal placental insertion Methods : We performed a retrospective quantitative monocentric study, in a tertiary perinatal care center, including all consecutive women with an abnormal placental insertion and who benefited from conservative treatment between January 2009 and June 2016. The exclusion criteria were treatment by cesarean hysterectomy and extirpative method. We compared two issues « conservative treatment with complica-tions » and « conservative treatment without complications » according to maternal criteria (risk factors of abnormal placental insertion, …), clinical criteria during preg-nancy (metrorrhagia, …) and criteria related to medical imaging (ultrasonographic signs, magnetic resonance imaging, …) during pregnancy. Results : Twenty nine patients were included in this study. There was a significant difference (p= 0.008) between the two groups for the « complete placenta praevia at antenatal ultrasound screening » variable. The overlapping character of the placen-ta on ultrasound screening during prenatal screening would be associated with an increase in maternal complications of postpartum conservative treatment. Conclusion : The overlapping character of the placenta on ultrasound screening during prenatal screening would be associated with an increase in maternal compli-cations of postpartum conservative treatment. This work calls for continuing this re-search with the same goal but with a wider population, coming from several hospi-tals, thus increasing the representativeness of the study. Knowing the risk factors for complications of conservative postpartum treatment would optimize the choice of surgical strategy when a placenta accreta has been detected.
