Pilotage et intégration d’un nouveau dossier médical d’urgence dans le système d’information du Centre Hospitalier de la Basse Terre

Résumé

The increase, of the coverage, the quality of the care and the security of the data of the patient in a public institution of health while managing the expenses is one of concerns of the hospital reforms of these last decades. The public institutions of health are in front of this reality. The improvement of the information system is one of solutions who could allow to answer these obligations. It is in the sense that in 2010 the hospital center of the Basse-Terre made a commitment in its project of integration of the Computerized patient record system Crystal Net. The successive deployment of several clinical modules allowed to cover a large number of functions, such as the pharmaceutical prescription, the plan of care, the management of the pharmacy, the management of the operating block, the appointments and many others. Today, we notice that our system of information built itself under the blow of the successive reforms, and evolved with software packages already in present which did not necessarily follow the standardization of the communication protocols of moment and this fact are not compatible with the emergency computerized medical system in present. So, the emergency department is at the heart of the process of care. Unfortunately, is provided with an emergency computerized medical system incompatible with the enterprise resource planning (ERP) Crystal Net. It is the reason why, to answer the improvement of the quality of the System of information generally speaking, for the patient, for the professionals and for the establishment. It is seemed necessary to replace the emergency computerized medical system by a highly compatible emergency computerized medical system.
L’augmentation, de la prise en charge, la qualité des soins et la sécurité des données du patient dans un établissement public de santé tout en gérant les dépenses est une des préoccupations des réformes hospitalières de ces dernières décennies. Les établissements publics de santé sont devant cette réalité. L’amélioration du système d’information est une des solutions qui pourrait permettre de répondre à ces obligations. C’est en ce sens qu’en 2010 le centre Hospitalier de la Basse Terre s’est engagé dans son projet d’intégration du Dossier Patient Informatisé (DPI) Cristal Net. Le déploiement successif de plusieurs modules cliniques a permis de couvrir un grand nombre de fonctions métiers, telle que la prescription pharmaceutique, le plan de soins, la gestion de la pharmacie, la gestion du bloc opératoire, les rendez-vous et bien d’autres. Aujourd’hui, nous constatons que notre SIH s’est construit sous le coup des réformes successives, et a évolué avec les progiciels déjà en place qui n’ont pas forcément suivis la standardisation des protocoles de communication du moment et de ce fait ne sont pas compatibles avec le DPI en place. Le service des urgences est au coeur du processus de soins. Malheureusement, il est pourvu d’un dossier médical d’urgence (SIU) incompatible avec le Progiciel de Gestion Intégré (PGI) Cristal Net. C’est la raison pour laquelle, pour répondre à l’amélioration de la qualité du SIH d’une manière générale, pour le patient, pour les professionnels et pour l’établissement, il est apparu nécessaire de remplacer le dossier médical d’urgence par un dossier médical hautement compatible.
