Sentence span : apport d’une nouvelle épreuve de répétition de phrases dans le diagnostic précoce et différentiel de l’aphasie progressive primaire logopénique

Abstract : Primary progressive aphasias (PPAs) are neurodegenerative diseases including several variants, for which the diagnosis requires a complete evaluation associated with imaging results. PPAs are still often misdiagnosed with other neurodegenerative diseases, such as Alzheimer's disease (AD), particularly considering the logopenic variant of PPAs. The aim of our study was to apply a new task of repetition of fourteen sentences of increasing length and complexity, to verify if it could lead to an early and differential diagnosis of the logopenic variant (lvPPA). Forty-six subjects underwent this test, including 20 PPAs (13 lvPPA, 5 semantic variant PPA and 2 nonfluent PPA), 10 AD subjects and 16 controls. We compared the scores and errors of each group on this task, and the PPAs subjects, and particularly the lvPPA subjects, had greater impairment of sentence repetition and made more substitution errors than the AD subjects or the controls. Additionally, we observed that lvPPA subjects made more semantic substitution errors than phonological ones. Finally, we analyzed the results of the subjects diagnosed as PPAs on the sentence repetition task of the Gremots, the "gold standard" in speech and language therapy (SLT) evaluation, and only 11 subjects out of 17 obtained a pathological score, which shows that subtle impairments of the phonological loop can go unnoticed on the Gremots task. Our task could therefore allow an early and differential diagnosis of the logopenic variant in its early stage. Thus, it is necessary to integrate such a sentence repetition task into SLT evaluations, and to pursue scientific research so that in practice speech and language therapists are better trained about PPA and can provide early and adapted treatment.
