La santé sexuelle et gynécologique des femmes ayant des relations sexuelles avec des femmes : réflexion sur la pratique des sages-femmes, gynécologues et médecins généralistes

Abstract : Introduction: There is not usually a specific prevention campaign regarding the sexual and gynecological health of women who have sex with women (FSF). However, they engage in more risky health behaviors than heterosexual women (e.g., drugs, tobacco, alcohol, and multiple partners) and are therefore more vulnerable to the development of certain cancers and sexually transmitted diseases. The stereotype of "sexual immunity", the standard of the heterosexual model, and ignorance of health factors specific to FSF interfere with them being able to get appropriate gynecological consultations. Objectives: The objective of this study is to understand how gynecological health providers support FSF women. Methods: 16 semi-structured interviews were carried out with midwives, gynecologists, and general practitioners between October 2020 and March 2021. Results / Analysis: The approach to sexuality by practitioners is hampered by their strong sense of intrusion into the intimacy of women. Sexual orientation is not clearly sought during an initial gynecological consultation. This could be explained by the weight of a professional practice strongly influenced by the presumption of heterosexuality, by the taboo to not discuss sexuality, and by health professionals’ lack of knowledge about women who engage in sexual behaviors with other women. Conclusion: The presumption of heterosexuality among patients and the lack of mastery of health professionals in approaching women about their sexuality make it almost impossible to include discussing homosexual practices as part of gynecological consultation. Strengthening sexual health training as well as the inclusion of sexual minorities during gynecological consultation are essential in order to offer them appropriate gynecological care.
