Polyarthrite Rhumatoïde avec ou sans antécédent d’infection à Chikungunya à La Réunion : étude PRASANCHIK

Abstract : Introduction: inflammatory rheumatism is a frequent complication of chronic Chikungunya virus (CHIKV) infection. They are dominated by rheumatoid arthritis (RA)-like pictures. There are few data to explain the relationship between rheumatoid arthritis and post-chikungunya rheumatism. Objectives: to compare the phenotype of RA patients with (CHIKV+) or without (CHIKV-) history of chikungunya and the positive serology versus suggestive symptomatology in the epidemic context and to study the imputability of chikungunya in the onset of RA Material and methods: retrospective inclusion of all patients followed at the North or South UHC with RA from January 2017 to December 2020. History of chikungunya was either biologically confirmed or based on suggestive symptomatology. Results: inclusion of 401 patients, 56 were CHIKV+ and 345 were CHIKV-. The serology was positive in 31/56 patients, 25/56 had suggestive symptoms. RA occurred 39.27 months after infection. In the CHIKV+ group, 53.57% had anti-CCP antibodies and 54.2% in the CHIKV- group. In univariate analysis, patients in the CHIKV+ group were less frequently anti-CCP positive (OR 0.42 [0.21 - 0.84] - p=0.011). In multivariate analysis, patients in the CHIKV+ group were less frequently under biologics (OR 0.48 [0.34; 0.67] - p=0.028) and DMARDs (OR 0.58 [0.42; 0.8] - p=0.09). Conclusion: fifteen years after CHIKV infection, RA patients in the CHIKV+ group showed less frequent auto-antibody positivity and immunosuppressive therapy than RA patients in the CHIKV- group. Post-Chikungunya rheumatism seem to be anti-CCP-independent.
